Download the proforma, fill it & e mail to permissioncell.moca@nic.in....
And then give a call at 011-24632950 (Extn 2873 )
....he will fwd the sanction to you by email.

Sectors on which General/blanket relaxation issued are at Annexure-I &
Annexure- II.
